The Ontario government announced eight new health teams today, including one for Grey and Bruce Counties.
An Ontario Health Team is a relatively new approach taken by the province to combine area health care providers into one team for more seamless transition of patient record and care plans.
Teams include providers from across health and community sectors, including primary care, hospitals, home and community care, mental health and addictions services, long-term care, and others.
There are now 50 health teams in Ontario, including the new Grey-Bruce Ontario Health Team, and the existing South Georgian Bay Family Health Team.
Minister of Health Christine Elliott confirmed in an announcement today there will be $9 million in funding from the province over the next two years to support the health teams.
Each health team will be provided with up to $1.1 million in one-time funding over the next two years, according to a news release from the provincial government.
The money can be used by each Ontario Health Team to support the COVID-19 pandemic response and coordinate programs that strengthen health system capacity by linking hospitals, primary care, home care, and community services.
